    -  NPWP (Nomor Pokok Wajib Pajak)
    -  SIUP (Surat Ijin Usaha Perdagangan)
    -  TDP (Tanda Daftar Perusahaan)
    -  Domisili Perusahaan
    -  KTP (Kartu Tanda Penduduk) of the person in charge
